Most clients can't answer what is a seemingly simple question "What is the purpose of your site?" As you've stated"...most companies still start with the design and work backwards." I guess it's true what they say "Common sense isn't very common!" :) Without the foundation you've described in this post any site design will fail to engage your audience in any meaningful interaction.
